Adds support for HT code-blocks to jpeg2000dec

Addresses comments against https://patchwork.ffmpeg.org/project/ffmpeg/list/?series=8661
  * simplify loop using av_log2()
  * document and simplify asserts
  * fix big endian and 32-bit builds
  * allow codestreams with both Part 1 and Part 15 code-blocks
