Some callers assume that item_name is always set, so this may be
considered an API break.

Ok.

We could fwiw announce that starting with an arbitrary major soname 
item_name can be NULL, and revert this revert.

Might be better to just replace it with a new field, since otherwise we
cannot signal this to the users.
